This is to introduce me to the Riverside Medical Center today to provide an interdisciplinary plan to address the major issues at the center. The objective of this plan is to enhance the coordination of care, since it is one of the causes of confusion and ineffective communication between departments whenever changing shifts. As a charge nurse, I ensure that the needs of the patients are addressed, the nurses assigned are handled, and communication across departments is effective.
