NURS FPX 4005 Assessment 4 Stakeholder Presentation
NURS FPX 4005 Assessment 4 Stakeholder Presentation Slide 1 Stakeholder Presentation My name is ______, and I am addressing you all. This is to introduce myself to Riverside Medical Center today and to provide an interdisciplinary plan to address the center’s major issues. The objective of this plan is to enhance care coordination, as this is a major cause of confusion and ineffective communication between departments during shift changes. As a charge nurse, I ensure that patients’ needs are addressed, assigned nurses are managed, and communication across departments is effective. The notion is expected to enable providers to enhance collaboration, improve patient outcomes, and retain nurses, thereby enhancing the performance of the healthcare system (Paguio et al., 2020). Slide 2 The goal of this communication plan is to apply structured tools and measures to reinforce cooperation between the administration and nursing staff at Riverside Medical Center during shift changes. The ultimate goal will be to enhance patient care by making it easier to exchange information, manage workloads more effectively, and make better decisions. The protocols are formulated with the involvement of all organizational members to ensure they feel included. This course of action aims to ensure that the needs of patients and medical staff are taken into account and that all parties are transparent and accountable (Brenner et al., 2022). The charge nurse claimed that failure to coordinate among healthcare providers may lead to issues with patient care handover at the end of the shift. Slide 3 Organizational Issue The urgency of improving the communication department at Riverside Medical Center is high, as workflow changes may slow patient care and raise safety concerns. Providing significant patient updates to administrative personnel is not always easy among nurses, which could affect the quality of hospital decisions and operations. To resolve these issues, nurses, administrators, IT specialists, and patient advocates will collaborate to establish and adhere to appropriate guidelines for healthcare communications. Taking this new direction will lead to improved treatment options and higher-quality care for patients (Keskinocak & Savva, 2019). The charge nurse noted that caring for patients is difficult to sustain, as healthcare providers are not well coordinated. Effective communication during shift change at Riverside Medical Center should be practiced by nursing and administrative staff to ensure appropriate treatment plans, continuous care, and patient responsiveness. Through these improvements, the hospital will be able to guarantee patient-centered care, which will translate into improved patient outcomes and quality of care (None et al., 2024). The proposed measures will enable patient information to be shared between departments with ease. Slide 4 Consequences of Not Addressing the Issue Shift changes are one of the causes of fragmented coordination in patient care at Riverside Medical Center. Nurses have difficulty reporting critical patient issues to administrative staff, resulting in delays in care, treatment, and decision-making. Disrupted communication negatively affects hospital functioning, endangers patients, and increases the risk of medical errors (Yaacoub et al., 2020). Addressing these issues will improve hospital functioning and patient care. Lack of effective communication between departments at Riverside Medical Center harms patients. The inability to promptly meet patients’ needs due to poor communication may lead to misunderstandings and result in longer-than-necessary responses, endangering patients in certain cases (Pryce et al., 2021). Communication breakdown compromises the quality of care, impairs the delivery of efficient, timely services, and threatens patients’ recovery. The solution to such issues will lead to improved patient care and more efficient healthcare. Slide 5 Relevance of Interdisciplinary Team Approach Clinical communication between departments during shift change is an essential factor at Riverside Medical Center, as an interdisciplinary approach integrates different perspectives to overcome fragmented care coordination. It must include nurses, IT specialists, administrators, and patient advocates in the team. Nurses share firsthand experiences of working with patients, administrators analyze the overall picture, and patient advocates help keep the focus on what is best in patients’ interests (Bhati et al., 2023). IT professionals are crucial to ensuring that necessary communication technologies are used effectively to streamline processes. Effective collaboration between these teams will enable them to achieve improved patient outcomes through well-organized work. Slide 6 Evidence-Based Interdisciplinary Plan Summary Objective The fundamental goal of this interdisciplinary plan is to establish effective communication between departments at Riverside Medical Center during shift changes. The primary objective is to connect an electronic handoff tool to the electronic health record (EHR) system to facilitate the transfer and organization of patient information to various departments with less effort. This is meant to prevent mistakes that could compromise patient safety. Regular interdisciplinary talks and communication practice should be used every day to facilitate the use of the EHR extension (Engle et al., 2021). It will seek to hold the healthcare process more accountable, promote effective teamwork, and result in smoother, coordinated care that facilitates the efficient operation of patients and the system. Working The communication improvement strategy is based on a continuous improvement model, with the focus now on implementing the electronic handoff tool. The team opens by defining key areas of communication failures between departments during transitions and establishing detailed objectives to be achieved. Their impact on collaboration and safety is evaluated after EHR changes are made by gathering data (Verma et al., 2021). These assessments improve best practices and standardize them, such as ensuring proper and complete updates and handover documents. Experiencing other disciplines and making necessary corrections will promote a culture where individuals are never done seeking improvement, thereby minimising communication problems and benefiting patients and employees. Role of the Interdisciplinary Team Riverside Medical Center comprises specialists representing different areas. The group of nurses, administrators, IT experts, and patient representatives applies their diverse experiences to enhance care coordination. Although administrators control resources and strategize organizational objectives, nurses identify and resolve communication discrepancies that affect patients. Patient advocates facilitate consideration of patients’ minds in the communication process, and IT specialists make the process easier by using technology to share information (Tamli & Sain, 2023). The knowledge collaboration workforce helps departments apply effective communication methods to facilitate proper and timely exchanges. These collaborative
